If your child requires medication during school hours, please complete the form below and email it to the school office - office@stedwards.blackburn.sch.uk

Once administered, the information will be emailed to you via Medical Tracker.

Please note: we can only administer medication where it specifies it be taken four times a day and so would need a dose during the day.

Asthma

To ensure our asthma records are upto date, if your child is prescribed an inhaler for a respiratory issue, and may need to use the school emergency inhaler, please complete the form below.

We do advise a spare inhaler, that is in date, always be left in school.
